The Home Care Packages Program was replaced by the Support at Home (SAH) program from 1 November 2025. Home modifications are now funded under the separate Assistive Technology and Home Modifications (AT-HM) scheme — a dedicated funding pool that does not draw from the client's regular ongoing services budget. Support at Home funding

AT-HM scheme funding tiers — 2025–26

The Assistive Technology and Home Modifications scheme provides separate, dedicated funding for home modifications — it does not reduce a client's regular ongoing services budget.

Important for case managers AT-HM funding is approved separately from the client's regular Support at Home budget — modifications do not reduce their ongoing services funding. Funding approvals are generally valid for 12 months, with a possible 24-month extension for complex works. How does the Support at Home AT-HM scheme work for home modifications?
From November 2025, all government-funded home modifications are delivered under the Assistive Technology and Home Modifications (AT-HM) scheme — a separate funding pool with three tiers: Low (up to $500), Medium (up to $2,000) and High (up to $15,000, or more with clinical evidence). Importantly, this funding is separate from the client's regular ongoing Support at Home budget — modifications don't eat into their daily services funding. We provide written quotes aligned to these tiers so plan approval is straightforward.
Can you work as a subcontractor under our Approved Provider agreement?
Yes — this is our preferred arrangement with Approved Providers. You maintain the client relationship and the provider agreement with Services Australia. We deliver the physical works, document everything with before and after photos and a completion report, and invoice you. You then invoice Services Australia or the plan manager. One call from you sets it all in motion.
What checks do your team hold?
All team members hold a current National Police Check and Working With Children Check. We carry $5M public liability insurance. A certificate of currency is available on request for your provider records. We can provide copies of all screening documents before our first job with your clients.
How do you handle clients who are anxious or hesitant about having tradespeople in their home?
This is very common with aged care clients and we plan for it from the first contact. We always call ahead to introduce ourselves and explain what will happen. The same team member is assigned to each client — so they see a familiar face every visit. We work slowly, explain every step, and never rush. If a client needs to stop or wants to observe, we accommodate that. Dignity and comfort are non-negotiable.
Do you provide documentation suitable for our compliance records?
Yes — every job generates before and after photographs, detailed job notes recorded in ServiceM8, a completion report, and an itemised invoice. All records are retained for 7 years. We can email documentation to you, the client's family, or any other nominated party within 24 hours of job completion.
Can family members book directly for their parent or relative?
Absolutely. Family members, power of attorney holders, or the client themselves can book directly. We provide a written quote aligned to the AT-HM funding tiers, which makes it straightforward to take to a case manager for plan approval. We're happy to communicate directly with family members throughout the process if that's preferred.
